KORONADAL CITY, SOUTH COTABATO – The Assistance to Individuals in Crisis Situations (AICS) program has reopened to residents of South Cotabato. The program is managed by the Office of the Provincial Social Welfare and Development (OPSWD) to provide quick support to individuals and families in crisis.
Sonia L. Bautista, Provincial Social Welfare and Development Officer, announced that all types of assistance under AICS are now available again.
“Ang mga assistance nga pwede na ma-avail sang mga pumuluyo amo ang Medical, burial, hospital, kag financial assistance.” Bautista said.
Bautista urged the public to complete the requirements before heading to the Provincial Capitol to speed up processing.
Based on PSWDO requirements, the required documents are the following:
For MEDICAL ASSISTANCE
Two (2) photocopies of the recently issued and signed doctor’s prescription
One (1) original copy of the price quote for medicines prescribed
One (1) photocopy of a valid ID of the patient and one of the representatives, or a barangay certification
For MORTUARY ASSISTANCE
Two (2) photocopies of the death certificate
Two (2) photocopies of the funeral contract bill
One (1) photocopy of a valid ID of the patient and one of the representatives, or a barangay certification
For HOSPITALIZATION Assistance
Two (2) photocopies of the final bill or statement of account
Two (2) photocopies of the confinement certificate, medical certificate, or medical abstract
One (1) photocopy of a valid ID of the patient and one of the representatives, or a barangay certification
For assistance with LABORATORY SERVICES
Two (2) photocopies of the laboratory request
Two (2) photocopies of the charged slip
One (1) photocopy of a valid ID of the patient
A valid ID of the representative or a barangay certification
Bautista also emphasized that the accepted valid government-issued IDs are:
Voter’s ID or Voter’s Certification
Senior Citizen ID
PWD ID
Solo Parent ID
UMID
Driver’s License
National ID
Bautista added that Governor Reynaldo S. Tamayo, Jr. has directed all relevant offices to make sure that every resident seeking assistance is immediately attended to using appropriate means, emphasizing that no applicant should leave the Provincial Capitol without receiving help from the provincial government.
